- Influenza: Saskatchewan residents age 5 and over with a valid Saskatchewan health services card.
- COVID-19: Saskatchewan residents age 12+ with a valid Saskatchewan health services card.
- Complete the consent form below to help speed up the administration process
- Self-screen for COVID-19 using the online tool. Be prepared to be screened by your pharmacy too.
- Wear loose clothing with short sleeves.
